Opportunity Criteria
Dignity Health Medical Group is actively seeking a highly patient-centric physician to serve as a Hospitalist for their unique hospital at home program based out of both Chandler Regional Medical Center and Mercy Gilbert Medical Center. Dignity Health Arizona has partnered with Contessa Health to form the Dignity Health Home Recovery Care Program (HRC). Patients requiring hospital-level admission that have low to moderate acuity are offered to participate in the Home Recovery Care Program. Patients have an in-person evaluation by the hospitalist in the hospital setting prior to enrollment - either in the emergency department or under observation status. The patient is then transported home and is met by an acute care RN (ACRN) where they receive their care in the comfort of their home. The following and subsequent days the hospitalist rounds on the patient virtually, with the assistance of the in-person ACRN. Treatment includes necessary lab draws, IV fluids/medications, imaging studies, DME, etc. This is further managed with biometric peripherals and Bluetooth enabled technology (i.e. stethoscope). When the patient no longer requires hospitalization, the hospitalist discharges the patient and documents a discharge summary. The patient is followed by the Dignity Health HRC team for 30 days and is reconnected to their primary care physician. JOB DESCRIPTION:
