ChristianaCare is hiring for a Nurse Practitioner of Physician Assistant to work full time in the New C astle Primary Care Practice. The selected candidate, in collaboration with the Family Medicine Physician s , will be responsible for the primary care of the patient population at the center , as part of a care team including physicians, other APCs, RNs, MAs and office staff. This practice also supports care at the New Castle Senior Center. Position includes some evenings, Saturday and on call coverage.

Principal Duties and Responsibilities

• Medical management of patients, in collaboration with the physician, to care, evaluate and manage patients in the Office, at the Senior Center and in Virtual Visits through assessment of symptoms, diagnosis, education and execution of treatment plans and follow-up.
• Acts as a liaison for patients, families, and health professionals, making appropriate referrals when treatment plan requires as well as coordinate transitions of care to maximize quality of life and functional status of the patient and his/her family.
• Fosters, maintains and strengthens community relationships that will help care for the patient's overall wellbeing.
• Utilizes evidence based medicine coupled with expert clinical knowledge to promote and provide education to the patient and family.
• Works with Nurse Manager to assure that practice offices are maintained according to JCAHO CLIA and OSHA Standards as well as incorporating CCHS Core Values in daily practice.

Education and Experience Requirements

• Master's Degree as an Advance Practice Nurse or as a Physician Assistant
• License or license eligible in the State of Delaware
• Prescriptive authority in the state of DE
• BLS required
• Experience with the geriatric population preferred
