AbsoluteCare is seeking a dynamic physician leader to put the principles of population health into action for our medically complex & socially vulnerable members. Through unique provider-payor relationships, AbsoluteCare is designed to care for society’s sickest patients. Prior to coming to AbsoluteCare, our patients are typically ill-served by the uncoordinated traditional health care system, bouncing between emergency rooms, hospitals, and specialists several times a year. Oftentimes, due to the effects of trauma and poverty, a significant portion of our population suffer from infectious diseases – including HIV, Hepatitis C, and complex wound infections. However, through our integrated care teams, including physicians, nurse care managers, social workers, behavioral health specialists, pharmacists, and community outreach workers, we transform the lives of our members. We offer complex, coordinated medical care through our medical teams with our onsite infusion center, full pharmacy, and lab & imaging suites. We also treat the social determinants of health, addressing housing instability, food insecurity, or simply loneliness through our integrated care team.

The Medical Director will have direct responsibility for the other physicians, nurse practitioners and physician assistants in the medical center. To succeed the Medical Director must have a strong clinical understanding of both primary care and infectious diseases (e.g., AAHIV certification). Working within our matrix organization, the Medical Director will collaborate closely with the National Directors of their respective disciplines to implement process improvements, enhance our standards of care and surpass discipline specific professional practice guidelines in our ambulatory intensive caring unit care model.  As a dyad partner with the Practice Administrator, the Medical Director will be responsible for all the medical center outcomes including utilization, quality outcomes and the overall operation of the medical center and providers. This role is instrumental to our fast-growing organization’s success, and requires a person who exhibits dynamism, humility, clinical acumen, the ability to work collaboratively, and strong leadership skills.

Specific Duties and Responsibilities

Clinical Leader of the Practice:

• Lead the medical center to deliver superb medical care utilizing a comprehensive "one stop shop" in a Patient Centered Medical Home (PCMH) model, along with Complex Care Management.
• Build relationships with and collaborate with health plan medical directors, local hospitals, specialists and other organizations to help meet the outcomes and decrease the fragmentation of care.
• Actively manage the team to track and meet contractual quality metric goals.
• Use technology and the electronic medical record to the highest capability in order to increase quality initiatives and efficiency within the medical center.
• Participate in multi-disciplinary meetings and recommend treatment alternatives designed to keep patients healthy or resolve on-going clinical issues.
• Working in a dyad model with the Practice Administrator, manage the Medical Staff and other AbsoluteCARE team members.
• Collaborate with National Directors across the company to implement process improvements, enhance our standards of care and surpass discipline specific professional practice guidelines in our ambulatory intensive caring unit care model
• Provide medical guidance to the Medical Staff, reviewing current medical practices and guidelines and implementing best evidence-based practices.
• Implement strategic medical operations and protocols in order to achieve company's contracted goals.
• Constantly evaluate the current medical operations and operations of the entire medical center to ensure compliance and contractual obligations.
• Strive to constantly improve the quality of care and outcomes for AbsoluteCARE patients.

Direct Complex Clinical Care (approx. 25%):

• Working with the assistance of your primary integrated care team to care for a small panel of patients
• Accept same day/next day or urgent appointments for patients from home or discharged from the hospital/ER/skilled nursing facility in need of intensified physician or case management/social work services to ensure acute episode has resolved and the patient is not admitted/readmitted.
• Able to work in a team environment (with social work, case management, behavioral health, pharmacy, nutrition, etc.), process feedback, and be open to discussing and implementing plans of care from other members of the team.
• Develop strong transitional care plans and able to help keep track of paneled patients along the whole continuum of care including discharges from the hospital, ER, skilled nursing facilities/nursing homes, and home care.
• Communicate and interact with alternate levels of care facilities (hospital, ER, skilled nursing facilities) to facilitate appropriate discharge to a lower level of care or back to the AbsoluteCARE Medical Center. Familiar with inpatient medicine and the discharge process to help facilitate discharges.
• Treat acute exacerbations of chronic conditions within the AbsoluteCARE center, i.e., Lasix for CHF, IVF for Dehydration/Renal Failure, IV antibiotics for various infections, Nebulizer/Solumedrol for COPD/Asthma etc. Possess a comfort level and the knowledge to differentiate when exacerbation can be treated within AbsoluteCARE and when a higher level of care needed.
