Acclaim Physician Group, a partner with JPS Health Network is seeking Family Medicine physicians to join our fantastic team!

The Department of Family Medicine staffs the JPS Primary Care services at 12 Patient Centered Medical Homes across Tarrant County. The medical home is a care delivery model where patients receive comprehensive services to address their physical and behavioral health. Primary, preventive, wellness, acute and chronic care are all available in one central location. Medical home care is patient-centered and coordinated among a team of doctors, nurses, assistants, pharmacists, educators and other providers. Combining services encourages coordination of care among providers and engagement by patients, who no longer need to travel to multiple locations for diagnostics such as X-rays and lab tests.

In addition to providing great care, JPS is also an urban teaching hospital with a long history of training physicians and medical students. 

Opportunities are available at the following clinics:

1. Diamond Hill -  Mon thru Thurs, 4 10 hour shifts per week.  Adult panel. Located in North Fort Worth
2. Southeast Medical Home - Scheduled TBD.  All ages.  Located in Arlington.

Applicant Requirements:

• Doctor of Medicine or Doctor of Osteopathy degree from an approved college of Medicine/Osteopathy.
• Board Eligible/Certified in Family Medicine
• Active Texas Medical License
• Unrestricted DEA
