Overview

Responsibilities

- Candidate will have the ability to utilize the nursing process to assess, diagnose, plan, implement and evaluate the plan of care for patients served. Through the model of Shared Governance, collaborate with all members of the health care team with a focus on promoting evidenced based care within a healing culture for the patient population served and their families.

- Candidate must have excellent communication and computer skills.

Minimum Experience Required:
- The selected candidate will perform comprehensive assessments for all patients; order medications, treatments, monitoring for efficacy and for possible side effects; order lab and other diagnostic tests, including follow up for results; communicate with the collaborating physicians on a regular basis per collaborative practice agreement; consult with patients and families regarding goals of care; follow patients through various transitions of care, to and from hospitals, facilities, and home, coordinating with in-hospital staff, facility staff , home care agencies to ensure continuity of care. In addition the candidate will coordinate chronic care programs for the practice; educate patients and families, specifically on disease process and medications; collaborate with the interdisciplinary team; provide comprehensive documentation in electronic medical record/ and /or paper medical records; assist patients and families with long term planning and resources for in home care; answer telephone calls from office, pharmacies, caregivers, patients and families, and facility staff; and manage acute and chronic illnesses.
