This position is located in the Primary Care service of the Stratton VA Medical Center. If selected, you will practice at the Bainbridge, NY Community Based Outpatient Clinic. You will function as an integral member of the Patient-Aligned Care Team and will be expected to work closely with team members, including nursing and administrative staff, in the coordination of patient care. You will participate in daily huddles with team members and in primary care systems redesign projects. To qualify for this position, you must meet the basic requirements as well as any additional requirements (if applicable) listed in the job announcement.
