Rockwood Clinic is seeking an experienced Nurse Practitioner or PA to participate in a multi-disciplinary team to augment the care delivered by our Primary Care providers. The APP in this position will be tasked with partnering with a small panel of patients and their families to develop care plans and focus on improved outcomes. 3+ year of clinical Nurse Practitioner experience

Post-acute care or long-term care experience

Washington State license by time of employment

Board certified at time of employment

DEA, NPI and prescriptive authority
