Mercy Health Physician Partners (MHPP) has an excellent full-time employment opportunity for a Board Certified or Board Eligible Family Medicine or Med-Peds physician to join our Innovative Primary Care team located in Grand Rapids, Michigan.

The selected candidate will join a robust practice of three physicians and three APPs and will be working with patients from newborns to geriatrics. The clinical care team currently consists of four RNs, nine 9 MAs, three front office employees, a chronic care manager and a Pharmacist. This practice is a non-traditional innovative office - always testing new ideas. Interested candidates must be comfortable with complex chronic disease (existing panel of approximately 1,400 patients).

The schedule for this outpatient only opportunity is Monday - Friday, 8 am - 5 pm and offers flexibility. Candidates should be comfortable with Virtual Medicine as this opportunity offers 50% virtual and 50% face-to-face patient care. Secured home Wi-Fi will be required for this opportunity. Call is by telephone only and is shared; one day per provider per month.

We are seeking a candidate who is interested in leading edge evaluation and process improvement related to technology, processes, and team development in a collaborative environment.

Candidates must have at least one year of ambulatory experience.
