Community Health Center (CHC) is a Federally Qualified Health Center located on beautiful Cape Cod with miles of beaches, natural beauty, and quaint villages. Cultural organizations and events surround this appealing area, yet the urban attractions of Boston and Providence are only 60 miles away.
CHC offers a full range of services, including primary and preventive care, access to walk-in clinic care, dental, behavioral health, pharmacy, radiology, vision, substance use disorder services and coordinated case management.
Recognized as a Level 3 Patient-Centered Medical Home, Community Health Center offers an innovative team-based, integrated model of care, along with telemedicine programs. CHC has a strong focus on community medicine, cultural competency, and outcomes management, supported by integrated behavioral health, complex care, lifestyle medicine and a remote patient monitoring program. We are rapidly growing and are building a diverse, driven and robust team while keeping patient satisfaction, quality care and staff satisfaction as a central priority.
